STEPS TO GET CERTIFIED

Step 1 | Schedule an appointment to get certified

During a 10-15 minute virtual appointment, a physician will evaluate your ailments and answer any questions you may have about medical marijuana treatment. If qualified, the doctor will certify you directly within the Department of Health Medical Marijuana Registry following the appointment.
You will need the following:

    • An electronic copy of a valid US government-issued photo ID and proof of DC residency address
    • An electronic copy of a clear, recent photo
    • Last four digits of the patient’s Social Security Number
    • Please note: In lieu of a recent photo, Hospice patients may submit a letter from the Attending of Hospice Record (AOHR) provider on hospice letterhead

Step 2 | Register Online

Visit the MMCC Registry website and click the “Create Account” link under the “Patients” heading

    • Complete the “Before You Begin” section to review and confirm that you have the required documents and information ready for upload
    • Complete the online application form, uploading the requisite files

Step 3 | Verify Email

After submitting your online application, you will receive a verification email

    • Click the link supplied in the verification email to verify your email address
    • You will receive another email after MMCC has reviewed your application, indicating it has been approved

Step 4 | Optional Final Step

Once approved by the MMCC, you can do the following:

    • Select a Caregiver if applicable by logging into the MMCC Patient Registry
    • Visit a registered provider to obtain a valid written certification
    • Purchase a Patient ID card (optional) for a fee of $50

QUALIFYING REQUIREMENTS & CONDITIONS

Qualification Requirements

Any individual who lives in the state of Maryland whose provider recommends medical cannabis as a treatment option for a qualifying medical condition is eligible to register with the Maryland Medical Cannabis Commission (MMCC) as a patient.

Qualifying Conditions
For adults and children, debilitating medical conditions include:
  • Cachexia
  • Anorexia
  • Wasting syndrome
  • Severe or chronic pain
  • Severe nausea
  • Seizures
  • Severe persistent muscle spasms
  • Glaucoma
  • Post-traumatic stress disorder (PTSD)
  • Chronic medical conditions which are severe and for which other treatments have been ineffective.

Qualification Requirements
You must be 21+ years of age and a resident of D.C. before you receive approval.

Qualifying Conditions

  • Glaucoma
  • Cancer
  • HIV/AIDS
  • Severe/persistent muscle spasms such as those in diseases like multiple sclerosis
  • A chronic condition that can’t be treated by ‘ordinary’ medical measures
  • Any situation where medical marijuana treatment is possibly beneficial
  • You may also qualify if you undergo medical treatment such as radiotherapy or chemotherapy
In reality, there are an extensive range of qualifying conditions. The Medical Marijuana Expansion Emergency Amendment Act of 2014 made things a little clearer. It states that patients could receive MMJ for “any condition for which treatment with medical marijuana would be beneficial, as determined by the patient’s physician.”

How do I get a patient certification?

It starts here, with an appointment to be evaluated from the safety and comfort of your home by one of our certifying Physician (MD, DO), Advanced Practice Registered Nurse / Nurse Practitioner (APRN/NP), Dentist, Physician Assistant, OR Naturopathic Physician.

What happens after I am certified by a doctor?

After the doctor certifies you in the Department of Health’s system, you will receive an email confirmation with your next steps on how to pay for your medical marijuana ID card.

When will I get my patient ID card?

Please allow 7 days for printing from the date you paid for your ID card and 14 days to receive it in the mail. Your card will be mailed to the address provided to the Department of Health.
